Redesigning Care / Support Pathways
The redesign of care and support processes is pivotal to delivering improvement and sustainable change at the local level. The emphasis is now on the individual’s needs, preferences and choices as driven through the whole ‘personalisation’ agenda. This requires services to be provided in a more integrated and seamless way.
We have helped clients to refocus their delivery of services through greater integration.

Developing People and Organisations
The changes in culture and practice that are needed to maintain the momentum of reform cannot be implemented overnight. Achieving the right competencies in your local workforce is a critical starting point.
In addition, effective Leadership and team working at every level will be required both to embed reform thinking and to implement joint initiatives productively at a local level. Building capacity and capability invariably hinges on the quality of your people and the ‘change champions’ you choose to drive initiatives forward.

Involving Local People
Across Government, the shared ambition is to put people first through a radical reform of public services. It will mean that people are able to live their own lives as they wish; confident that services are of high quality, are safe and promote their own individual needs for independence, well-being, and dignity.
Reform is all about integrating services around the individual person. Local people will not only be knowledgeable about the health and healthcare choices available to them, their feedback will drive initiatives forward. The need to develop inclusive processes and engage local people and partner organisations from the outset is the key, is central to our approach in developing any strategic approach to commissioning and provision.
We can support you either as an integral part of a programme of work or as a separate initiative.
